Falls are a major problem in public health since they are an important cause of morbidity and mortality. To evaluate the risk of fall and prescribe preventive interventions may be a challenging task.
The objectives of this study are to summarize the most relevant information on the topic "falls in the elderly" and to give a critical view and practical clinical approach on this topic.
In March 2022, a search of Pubmed database was performed, using the terms "fall elderly", fall prevention", "fall risk", with the following parameters: five years, review, systematic review, meta-analysis, practice guidelines.
There are several risk factors for falls that can be grouped in different areas (psychosocial, demographic, medical, medication, behavioral, environmental). The clinical evaluation of an older adult prone to falls must include identification of risk factors through history and examination and identification of risk of falls through an assessment tool such as gait velocity, functional reach test, timed up and go, Berg balance test, and miniBEST test. Fall prevention strategies can be single or multiple, and physical activity is the most cited. Technology can be used to detect and prevent falls.
A systematic approach to the older patient in risk of falls is feasible and may impact fall prevention.
As quedas são um grande problema de saúde pública, uma vez que são uma importante causa de morbidade e mortalidade. A avaliação do risco de queda e a prescrição de intervenções preventivas podem ser tarefas desafiadoras.
Estudo visa sintetizar as informações mais relevantes sobre o tema “quedas em idosos” e apresentar uma visão crítica e uma abordagem da prática clínica neste tema.
Em março de 2022, foi feita uma revisão bibliográfica no Pubmed, utilizando-se os termos “fall elder”, fall prevention”, “fall risk”, com os seguintes parâmetros s: 5 anos, revisão, revisão sistemática, meta-análise, diretrizes práticas.
Há inúmeros fatores de risco para quedas que podem ser agrupados em diferentes domínios (psicossocial, demográfico, condições clínicas, uso de medicamentos, comportamental, ambiental). A avaliação clínica de um idoso propenso a queda deve incluir a identificação de fatores de risco por meio da história e exame físico e identificação do risco de queda por meio de um instrumento de avaliação como velocidade da marcha, teste de alcance funcional, timed up and go, teste de equilíbrio de Berg, escala miniBEST. As estratégias de prevenção de quedas podem ser únicas ou múltiplas, sendo a atividade física a mais citada. Tecnologias podem ser usadas para detectar e prevenir quedas.
Uma abordagem sistemática ao paciente idoso em risco de queda é possível, deve ser realizada e pode impactar na prevenção de quedas.

Falls in older adults: a practical approach.

To estimate the prevalence of individual chronic conditions and multimorbidity among adults admitted to hospital in countries in sub-Saharan Africa.
We systematically searched MEDLINE®, Embase®, Global Index Medicus, Global Health and SciELO for publications reporting on patient cohorts recruited between 1 January 2010 and 12 May 2023. We included articles reporting prevalence of pre-specified chronic diseases within unselected acute care services (emergency departments or medical inpatient settings). Of 6976 identified studies, 61 met the inclusion criteria, comprising data from 20 countries and 376 676 people. None directly reported multimorbidity, but instead reported prevalence for individual conditions. Among medical admissions, the highest prevalence was human immunodeficiency virus infection (36.4%; 95% CI: 31.3-41.8); hypertension (24.4%; 95% CI: 16.7-34.2); diabetes (11.9%; 95% CI: 9.9-14.3); heart failure (8.2%; 95% CI: 5.6-11.9); chronic kidney disease (7.7%; 95% CI: 3.9-14.7); and stroke (6.8%; 95% CI: 4.7-9.6).
Among patients seeking hospital care in sub-Saharan Africa, multimorbidity remains poorly described despite high burdens of individual chronic diseases. Prospective public health studies of multimorbidity burden are needed to generate integrated and context-specific health system interventions that act to maximize patient survival and well-being.

Parmi les admissions à l'hôpital, le plus haut taux de prévalence concernait: les infections au virus de l'immunodéficience humaine (36,4%; IC de 95%: 31,3–41,8), l'hypertension (24,4%; IC de 95%: 16,7–34.2), le diabète (11,9%; IC de 95%: 9,9–14,3), l'insuffisance cardiaque (8,2%; IC de 95%: 5,6–11,9), les maladies rénales chroniques (7,7%; IC de 95%: 3,9–14,7) et les accidents vasculaires cérébraux (6,8%; IC de 95%: 4,7–9,6).
La multimorbidité des patients se rendant à l'hôpital en Afrique subsaharienne demeure peu documentée, en dépit de la lourde charge que font peser les maladies chroniques individuelles. Des études prospectives de santé publique sont nécessaires à ce sujet pour déclencher des interventions du système de santé adaptées au contexte et améliorer ainsi les chances de survie et le bien-être des patients.

Incluimos artículos que informaban sobre la prevalencia de enfermedades crónicas predefinidas en el marco de los servicios de cuidados intensivos no seleccionados (salas de urgencias o centros médicos hospitalarios). No se aplicaron restricciones lingüísticas. Generamos estimaciones sobre la prevalencia utilizando un metaanálisis de efectos aleatorios, junto con intervalos de confianza del 95%, intervalos de predicción del 95% y estadística

Entre los ingresos médicos, la prevalencia más alta correspondió a: la infección del virus de la inmunodeficiencia humana (36,4%; IC del 95%: 31,3–41,8), hipertensión (24,4%; IC del 95%: 16,7–34,2), diabetes (11,9%; IC del 95%: 9,9–14,3), insuficiencia cardíaca (8,2%; IC del 95%: 5,6–11,9), enfermedad crónica renal (7,7%; IC del 95%: 3,9–14,7) e ictus (6,8%; IC del 95%: 4,7–9,6).
Entre los pacientes que buscan atención hospitalaria en el África Subsahariana, la multimorbilidad sigue estando escasamente descrita, a pesar de las elevadas tasas de enfermedades crónicas individuales. Los estudios prospectivos de salud pública que versan sobre tasas de multimorbilidad son necesarios para generar intervenciones integradas y adaptadas a las circunstancias por parte del sistema sanitario, para poder aumentar así la supervivencia y el bienestar de los pacientes.
